What is Perineal Urethrostomy Surgery?

perineal urethrostomy (PU) is a surgical procedure commonly performed on male cats with urinary obstructions. Let’s explore the details:

Indications:

    • Urinary Obstruction: PU is recommended when a urinary obstruction occurs within the penis and cannot be resolved through medical therapy.
    • Recurrent Obstructions: Some male cats experience frequent obstructions. Even if unblocked, they may benefit from PU to decrease the likelihood of future obstructions by creating a larger urinary opening.
    • Severe Urethral Trauma: In cases of traumatic injury to the penis, PU allows urine to exit the body before reaching the damaged part of the urinary tract, preventing complications like scar tissue or infection.

Procedure:

    • Surgical Approach: PU creates an opening in the perineum—the space between the rectum and scrotum.
    • Penis Removal: The penis is completely excised.
    • Wider Urethral Opening: The urethral opening is enlarged, allowing urine, stones, and sand to pass more easily.
    • Scrotal Urethrostomy: For intact males, PU is often combined with castration to prevent future obstructions.

Postoperative Care:

    • Cats should be kept calm and inactive during recovery.
    • Bleeding from the surgical site may occur for up to 10 days.
    • Monitoring for any signs of complications is essential.
