What is Soft Palate Resection (Staphylectomy)?

Soft palate resection, also known as staphylectomy, is a surgical procedure performed to address elongated soft palates in brachycephalic breeds. These breeds, such as English Bulldogs, French Bulldogs, Pugs, and Boston Terriers, are prone to upper airway abnormalities collectively known as brachycephalic syndrome.

Indications:

  1. Elongated Soft Palate:
    • Brachycephalic dogs often have elongated soft palates that obstruct their airways.
    • The excess length of the soft palate can lead to noisy breathing, exercise intolerance, and heat sensitivity.
    • Surgical resection is indicated when the soft palate extends significantly beyond the tip of the epiglottis.

Procedure:

  1. Preoperative Evaluation:

    • The patient is placed under general anesthesia for accurate assessment of the soft palate and other upper airway structures.
    • The soft palate is considered elongated if it extends 3 mm or further past the epiglottis tip.
    • Correct positioning during evaluation is crucial to avoid altered landmarks due to traction on the tongue or presence of an endotracheal tube.
  2. Surgical Technique:

    • The excess portion of the soft palate is resected using various methods:
      • Traditional Sharp Resection: Using a scalpel blade or Metzenbaum scissors.
      • Bipolar Sealing Device (BSD): A cautery tool that minimizes postoperative swelling.
      • Fine-Tip Electrocautery Unit: Provides precise tissue removal.
      • CO2 Laser: Reduces pain, inflammation, and scarring.
    • The goal is to create a patent airway by removing the obstructive tissue.
  3. Postoperative Care:

    • Monitoring: Close observation for signs of complications (swelling, bleeding, or altered breathing).
    • Improved Breathing: Most patients experience easier breathing and reduced distress.
    • Gagging and Retching: If these symptoms were present, they should improve significantly.
